We welcome patients from within the Practice catchment area, to see if you are in our catchment area click on the Find My GP link below.
We do not accept patients to our list outside this area and if you move out of our area you will be asked to register with a Doctor nearer your new address.
To register at Old Hall Surgery please use this form and this will be sent to the surgery.
New patients must undergo a new patient medical. This appointment will be arranged by a member of the Reception Team at your earliest convenience and you will be asked to bring a urine sample with you.
If you are on any regular medication you will need to speak to the Clinical Pharmacist to have this made available as a repeat prescription.
If you are a carer please also complete the carer registration form.
